Our team is here to not only put your child at ease during their appointments, but provide parents with everything they need to navigate their child’s treatment. To help make your child’s visits go smoothly, we kindly request that you bring your photo ID, as well as your medical and dental insurance details, to every appointment. Patient Information—Patient Forms
At Pediatric Dentistry of Fredericksburg, we understand that your time is precious. That’s why we offer patient information such as new patient forms online! Simply download our new patient forms, fill them out at home and bring them with you to your child’s appointment to save time during check-in. Alternatively, you can fill out the forms entirely online and submit them to our office ahead of your child’s appointment. If you have any questions, please call our office at 540 371-3222.
Insurance & Billing
To ensure a positive and easy experience for our patients, we gladly offer assistance with insurance-related matters. Our team will work with you to verify your dental insurance benefits and submit claims on your behalf to your insurance provider. While our team will do everything in our power to maximize your insurance benefits, please note that the approval of insurance claims is contingent on your particular insurance provider—not our practice. Therefore, even with insurance, a remaining balance may remain on your account. We kindly ask that any payments not covered by your dental insurance for the treatment received will be due at each appointment. We accept cash, personal checks, MasterCard and Visa.
